Discussion was held on the topics of current enrollment, that MAT certification shall expire in October, and current staffing.
The following inspection took place between the hours of 11:45 am through 1:30 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Fairfax, Virginia. At the start of this unannounced monitoring inspection there were a total of 10 children (28 points) with 3 caregivers. A total of 3 children?s files and 5 staff/household member?s files were reviewed. The children were engaged in the following: music, song, games, large motor skills outside, diaper changing, bathroom break, hand washing, clean up after lunch, nap time, and other organized activities. Standard #: 22VAC40-111-60-B Description: Based on review, 1 of 3 children's files reviewed indicated that not all of the required information for each child was on file. Evidence - 1. On the date of inspection (03/31/2016), the name and policy number of Child 5's medical insurance was not available for review. 2. The area of the child's record was left blank. Plan of Correction: This information shall be obtained.
Standard #: 22VAC40-111-90-C Description: Based on review, 1 of 3 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child who was not exempt from the immunization requirements according to subsection B of this section once every six months for children under the age of two years. Evidence - 1. On the date of inspection (03/31/2016), the most current immunization page on file for Child #2 was dated - 09/08/2015. 2. On the date of inspection, based on the information on file, Child #2 was under the age of two years. Plan of Correction: An updated immunization page shall be obtained for Child #2.
