22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced renewal inspection was initiated today at 12:10pm. The physical plant, 2 caregiver files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 12 children, totaling 31 points, under the supervision of 2 caregivers. During the inspection, children were observed cleaning up for and eating lunch, resting at nap time, putting puzzles together and enjoying free play. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served pasta with carrots and broccoli, bananas and milk. The Provider is not certified in MAT (medication administration training) and there are no children with medication needs while in care. Both caregivers are certified in CPR and First Aid. Standard #: 22VAC40-111-100-A Description: Based on file review, 1 out of 4 children's files does not have a physical examination within 30 days of attendance. Evidence 1. Child #3 began care on 9/8/15. There is no physical examination on file. Plan of Correction: The Provider has already requested this from the parents. She will request it again.
Standard #: 22VAC40-111-120-B Description: Based on file review, an assistant's file does not have all required information. Evidence 1. Caregiver #2 does not have the following information on file: - Name address and telephone number of a person to be notified in an emergency. - Documentation that two or more references as to character and reputation as well as competency were checked before employment. Plan of Correction: The reference checks will be completed and the caregiver will provide the emergency contact in writing.
Standard #: 22VAC40-111-90-C Description: Based on file review, 1 out of 4 children's files does not have additional immunizations once every six months up to the age of two years. Evidence 1. Child #3 is 10 months old. The most recent immunization record was completed when the child was 12 weeks old. Plan of Correction: The Provider has already requested this from the parents. She will request it again.
