Comments:
Today an unannounced renewal inspection was conducted beginning at 11:30am. There were 8 children (26 points) present today with the Provider and two assistants. Children were observed having freeplay, eating lunch, and napping during this inspection. Today we discussed many areas in the home that pose safety hazards based on shelves being stored on top of each other, full of supplies and equipment. The LI will return after two weeks to verify changes have been made before making a recommendation on the renewal of the license. This inspection, including the exit interview, ended at 2pm. Violations:
Standard #: 22VAC40-111-730-D
Description: Based on direct observation and discussion, household medications, including prescription and over the counter medications, are not locked away out of reach of children. Evidence: Prescription medications, over the counter medications, and vitamins for household members are on the counter in the kitchen, where children have access to and not locked away.

Plan of Correction: We will move the medications so that they are all locked away from children.

Standard #: 22VAC40-111-240-A
Description: Based on direct observation, areas inside the home pose safety hazards for children. Evidence: On the second floor of the home, along a wall that leads to a stairway upstairs, there are three wooden shelves for storage. On to of one of the wooden shelves is another shelf, made of metal, with four metal posts that do not completely fit on top of the wooden shelf. The combination of the wooden shelf with the metal shelf on is approximately 8 feet tall. Because the metal shelf does not completely fit all four of it's posts on the wooden shelf, and is not secured to the wall, this combination is unsturdy and poses a hazard if bumped in to by a child. Two cribs in the basement area are not being used for children in care. Inside the cribs are activity stations, a swing for infants, and various other equipment piled inside, up over the tops of both cribs. On the diaper changing table in the basement area, there is a plastic shelf stacked on top used for storage. The diaper changing table is not in use, however, the additional shelf on top of it is not stable and poses a hazard.

Plan of Correction: We will get make sure any shelves that are stacked on top of another shelf are put at floor level. We will get rid of the cribs we are not using. We will donate all extra supplies and equipment we don't use and rearrange so that children have more space.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, three adult records (#1, 2 and 4) are missing updated sworn statements dated within 3 years of the most recent ones. Evidence: The most recent sworn statements filled out by adults #1,2 and 4 are dated March, 2013.

Plan of Correction: We will review and update the sworn statements. We will update them every three years.
