An unannounced monitoring inspection was conducted today from 11:00 - 12:30. There were 9 children present = 24 points with three caregivers. The children watched a video before lunch, ate lunch, listened to stories, played with blocks and watered their plants. The exit interview was conducted with the licensee. The violation notice will be e mailed for completion within 10 days.

Standard #: 22VAC40-111-960-E
Description: Based on observation, the licensee did not label all of the children's bottles with their full name and the date. Evidence - One bottle was dated but not labeled with a name and a second bottle was labeled only on the bottle top so that when the top was removed, it was unlabeled.

Plan of Correction: I have asked the children's parents to correctly write their children's names on their bottles.

Standard #: 22VAC40-111-50-A
Description: Based on a review of the attendance sheets provided, the licensee did not keep a written record of children in attendance. Evidence - The two attendance records provided by the licensee were both incorrect. One record showed a blank for wednesday the 30 of March and the other one showed a date of 3/30 as being a Thursday.

Plan of Correction: I have made the corrections noted.

Standard #: 22VAC40-111-180-A
Description: Based on a review of staff records, the licensee did not obtain an updated tuberculosis screening every two years for one staff member. Evidence - The record for assistant #1 contained a tuberculosis screening that expired on 2/17/2016.

Plan of Correction: I have notified my assistant and she will update her tuberculosis shot.

Standard #: 22VAC40-111-480-B
Description: Based on observation, the licensee allowed children to use toys and equipment that were not appropriate for their age. Evidence - There is a zebra rocker in the child care area labeled for use by children ages 9 months - 2 1/2 years. It was observed being used by four year old children. There was one child observed playing with a matchbox type car which is intended for use by children three years and older and was accessible to all of the children including a two year old.

Plan of Correction: The zebra rocker will be stored out of reach of children that are older than 2.5 years old, and will only be brought out for children between ages 9 months - 2.5 years old when they want to play with it, and while under supervision.
