Comments:
An unannounced monitoring inspection was conducted today from 11:10 - 12:10. There were 4 children = 12 points with two caregivers. The children ate lunch, played with blocks, danced and listened to stories. The licensee now has a dog. The dog is current on all immunizations. The exit interview was conducted with the licensee. The violation notice will be e mailed for completion within 10 days.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of children's records, the licensee did not obtain physical examinations for two children. Evidence - The records for children # 2 and #3 did not contain physical examinations.

Plan of Correction: Got the physical exam.

Standard #: 22VAC40-111-110-B
Description: Based on a review of children's records, the licensee did not obtain all of the required information on the physical examination for one child. Evidence - The record for child #4 contained a physical examination that was not signed by a doctor or medical personnel.

Plan of Correction: Got the physical exam: got it signed.

Standard #: 22VAC40-111-480-D
Description: Based on observation and information from the licensee, there was a piece of equipment being used incorrectly. Evidence - There was a "Snuggle Nest" in use by an 8 month old. The manufacturers instructions state that it is to be used only for children birth - 4 months.

Plan of Correction: Return to the family.
