54.1-3408 Provider must be MAT certified to administer prescription medication.

Comments:
An unannounced monitoring inspection was conducted from 11:30 am - 12:30 pm. There were 6 children in care being supervised by the provider and one assistant ages 8 months, 18 months, 2 years, 3 years, 4 years, and 5 years for a total of 14 points. The children were observed participating in free play and preparing for lunch. Lunch served today is tuna, rice with tomato sauce, carrot and raisin salad, and milk. The provider, one caregiver, and two household member record's were reviewed. Six children's records were reviewed. The provider is current in CPR and First Aid training. Standard #: 22VAC40-111-100-A
Description: Based on review of 6 children's records it was determined that not all records contained an up to date physical exam within 30 days of enrollment. Evidence: Child A with a start date of 12/31/2013 did not have documentation of a physical on file.

Plan of Correction: The provider will request an up to date physical from the parents of child A.

Standard #: 22VAC40-111-180-A
Description: Based on review of one caregiver record it was determined that the provider did not obtain a TB (Tuberculosis) screening every two years from caregivers. Evidence: The most recent TB screening for Staff A was dated 11/04/2013.

Plan of Correction: Staff A will obtain an up to date TB screening and submit the results to the provider.
