An unannounced monitoring inspection was conducted from 11:00 a.m.-12:00 p.m. with the provider. Seven children, ages 2 years, 2 years, 3 years, 3 years, 4 years, 5 years, and 5 years old, were in the care of the provider for a total of 12 points. Children were observed eating lunch and coloring. Lunch served today was cheeseburger sliders, peas, applesauce, chips, and milk. The provider is current in first aid and CPR. The last emergency escape drill was 4/24/16. Four children's records were reviewed. Two household members' records were reviewed and in compliance. Standard #: 22VAC40-111-90-A Description: Based on review of four children's records, the provider did not obtain documentation of immunizations for a child prior to the child's first attendance. Evidence: Child A's (enrolled 4/4/16) record did not include documentation of immunizations. Plan of Correction: The provider will notify the parent and acquire the immunization documentation.
