54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted with provider. There were three day care children and one resident child present today totalling eight points. The children were having rest time during today's inspection. Three children's files were reviewed - please see Supplemental Summary. Provider has paperwork for two emergency medications, which were not needed today and not on site. Provider has Medication Administration Training valid until 11/13/2018. First aid and CPR certification is valid until 3/7/2017. There are many age appropriate books, games, toys, and activities available to the children. Standard #: 22VAC40-111-100-A Description: Based on review of three children's files, the provider did not have a physical examination on file for a child within 30 days of child's first attendance. Evidence: Child A (date of enrollment 6/8/2015) does not have documentation of a physical on file. Plan of Correction: Provider texted the parent to request documentation of a physical.
