54.1-3408 Provider must be MAT certified to administer prescription medication.
An announced initial inspection was conducted on April 28, 2016. The provider and her assistant were present with 5 children. (10 points) The children played with toys during the inspection and later washed their hands for lunch, which was being served as the inspection ended. The entire home was observed and it was noted that the children will use the kitchen, family room, front room, one bedroom, and one bathroom. The deck and back yard will be used for outdoor play. The family pet, a dog, is secured away from the children. The provider will not provide transportation. A recommendation will be made to issue a conditional license after the documentation from local zoning is on file. The capacity will be for 12 children and the age range will be 2 months through 12 years.
Standard #: 22VAC40-111-760-A-1 Description: Based on observation, the family day home did not have the required triangular bandages with the first aid kit. Plan of Correction: The bandages will be added to the kit.
Standard #: 22VAC40-111-770 Description: Based on observation, the family day home did not have a portable battery operated weather radio with extra batteries. Plan of Correction: A radio will be added to the emergency supplies.

Evidence: The 911number, poison control number, and person responsible for emergency back up were not posted. Plan of Correction: The information will be posted.
Standard #: 22VAC40-111-940-B Description: Based on observation the family day home did not have an operable thermometer available to monitor refrigerator and freezer compartment temperatures. Plan of Correction: A thermometer will be purchased.
