Today an unannounced monitoring inspection was conducted beginning at 9:45am. There were 5 children (10 points) present with the Provider and two assistant caregivers. Children were observed playing with train sets, having a tea party and preparing to work on learning "long and short". Three caregiver records were reviewed for updated and new documentation. One child's record was reviewed. There are no children enrolled with medications at this time. This inspection, including the exit interview, ended at 11:45am. Standard #: 22VAC40-111-120-B
Description: Based on record review, one adult caregiver (#2) is missing complete required documentation in their record. Evidence: Caregiver #2 is missing documentation of the following prior to their first day of employment: date of hire, emergency contact information, documentation of education and experience, and documentation of two reference checks completed.

Plan of Correction: We will have the new caregiver fill out these forms and provide us with the documentation this weekend.

Standard #: 22VAC40-111-200-A
Description: Based on discussion and record review, there is no documentation of orientation training for a new caregiver (#3). Evidence: The new caregiver (#3) has had the orientation training, however, there is no documentation supporting this.

Plan of Correction: I will document the training we did when she first started and place it in their file.
