An unannounced monitoring inspection was initiated today at 12:50pm. The physical plant, 3 caregiver/household member files, 4 children's files, caregiver training, children's activities, emergency procedures, drills and supplies were inspected today. There were 7 children, totaling 15 points, under the supervision of 2 caregivers. During the inspection, children were observed playing outside, cleaning up for and resting at nap time. The interactions between the children and caregivers were seen to be positive and constructive in nature. The children bring their lunches from home each day. The Provider is not certified in MAT (medication administration training) and there are no children with any medication needs at this time. Both caregivers are certified in CPR and First Aid. Standard #: 22VAC40-111-830-A Description: Based on observations, the Provider does not have documentation to support emergency evacuation procedures being practiced every month. Evidence 1. There is no documentation of an evacuation drill having been completed in March 2016. Plan of Correction: This will be completed and documented each month.
Standard #: 22VAC40-111-940-B Description: Based on observations, there was no thermometer in the refrigerator to monitor the temperature. Plan of Correction: This was found during the inspection and placed in the refrigerator.
Standard #: 22VAC40-111-100-A Description: Based on file review, 1 out of 4 children's records does not have documentation of a physical examination on file. Evidence 1. Child #1 does not have a physical on file. Plan of Correction: The Provider will ask the parent to bring this into the FDH.
Standard #: 22VAC40-111-120-B Description: Based on file review, a caregiver's file does not have all required documentation. Evidence 1. Caregiver #3 does not have the following information: - Name address and telephone number of a person to be notified in an emergency. - Documentation that two or more references as to character and reputation as well as competency were checked before employment. Plan of Correction: The Provider will complete this and document it.
Standard #: 22VAC40-111-90-C Description: Based on file review, 3 out of 4 children's files do not have additional immunizations updated every six months up to the age of 2 years. Evidence 1. Child #2's (2 yrs) most recent immunization record was updated when the child was 12 months old. 2. Child #3's (21 months) most recent immunization record was updated when the child was 13 months old. 3. Child #4's (2 yrs) most recent immunization record was updated when the child was 19 months old. Plan of Correction: The Provider will ask the parents to bring these into the FDH.
Standard #: 22VAC40-111-180-A Description: Based on file review, 1 caregiver/household member does not have a TB test/screening completed within the past two years. Evidence 1. Caregiver/household member #2's most recent TB test/screening was completed on 2/21/13. Plan of Correction: This caregiver is in the process of having this completed.
