An unannounced monitoring inspection was conducted today. There are 3 children present (10 points) with 2 adults. One child's file was reviewed. See violation notice for missing information. Two children have no file for review, even children on a drop in basis. Children were observed being fed, playing and sleeping. Please monitor the refrigerator temperature to ensure it goes down after the dial was adjusted. Description: Based upon a review of the refrigerator thermometer, the refrigerator temperature is registering at 48 degrees.

Plan of Correction: The temperature was lowered, and I will monitor it to make sure it goes to below 40 degrees.

Description: Based upon direct observation, infant bottles are not labeled and dated.

Plan of Correction: I will ask the parents to labeled and date the bottles.

Description: Based upon Provider's statements, an attendance sheet has not been used for children. Evidence: 1 child began attending 5/2/16, with no attendance sheet in use.

Plan of Correction: I will use the attendance sheet for the children.

Description: Based upon provider's statement, each child does not have a file available for review. Evidence: Children 1 & 2 do not have a file available.

Plan of Correction: I will give the parents the paperwork today.

Description: Based upon a review of files, one child's file is incomplete. Evidence: 1 of 1 files reviewed was missing a second emergency contact's complete address information.

Plan of Correction: I will ask the parent to complete the address.

Standard #: 22VAC40-111-70-A
Description: Based upon a review of files, one child's file does not contain documentation of required policies and procedures. Evidence: Child 1's file doe not contain documentation of written acknowledgment of the decision to administer medication.

Plan of Correction: I will ask the parent to sign the form.

Description: Based upon a review of files, children's files do not contain documentation of immunization prior to the first day of attendance. Evidence: Children 1-3 do not have any documentation of immunizations on file.

Plan of Correction: I will ask the parents to bring in the shot records.

Description: Based upon direct observation, the cribs do not have tight fitting bottom covers. Evidence: The sheets in use on the cribs are not tight fitting sheets.

Plan of Correction: I will purchase new sheets today.
