The Liability Insurance Declaration form was provided as requested. The Provider reported that the form will be updated with each parent. The current documentation will be maintained in the records of each child in care.
An unannounced monitoring inspection was conducted in the Family Day Home on May 6, 2016. Four children were present with the Provider and the Assistant. Based on the ages of the children, the total points were nine. The Provider reported eight children are currently on roll. The children were observed in the child-care area playing and coloring. The children were also observed during the lunch meal. After lunch, the children rested/napped on their mats. A sample of three children's records were reviewed. A discussion regarding practiced emergency drills was held with the Provider. The required monthly evacuation drills (12) and the required annual shelter-in-place drills (2) may be documented on the model form. The LI also discussed the coverage amounts listed by the Provider on the liability insurance disclosure. It was verified by the Provider that the documented coverage amounts are not current and the current amounts meet the established requirement. If you have any questions, please call (540) 309-2310. Standard #: 22VAC40-111-750-A Description: Based on the LI's observations, the Provider failed to ensure a diaper ointment was labeled with the child's name. Evidence: A diaper ointment was observed on the kitchen counter, the ointment container was not labeled for use for a specific child. A written parental authorization was reviewed in the record of child #1. The Provider labeled the ointment with the child's name upon notice. Plan of Correction: The Provider will label all topical skin products to identify the child's name at the time a parent brings the ointment and signs the authorization form.
Standard #: 22VAC40-111-750-B Description: Based on the LI's observations, the Provider failed to ensure a topical ointment was kept inaccessible to children. Evidence: A diaper ointment was observed lying on the kitchen counter in the child care area where preschool-age children were observed . Plan of Correction: All ointments will be stored in an area out of reach of the children.
Standard #: 22VAC40-111-780-B Description: Based on a sample review of children's records, the Provider failed to annually review with the parent the emergency contact information required in (22 VAC 40-111-60-B2) to ensure the information is correct. Evidence: A sample of children's records was reviewed by the LI. Two of three records (children 2 & 3) were last updated on 05/01/15. Plan of Correction: The Provider will review the children's records with the parent to verify information is current. Documentation of signed acknowledgment will be in each child's record.
Standard #: 22VAC40-111-830-A Description: Based on reviewed documentation of the emergency evacuation procedures, the Provider failed to practice a monthly drill as required. Evidence: The LI reviewed documentation of the practiced emergency drills. The document did not indicate that an evacuation drill was practiced in the month of March, 2016. The document noted a shelter-in-place drill was conducted in March. Plan of Correction: The Provider will practice an evacuation drill monthly with the children. Documentation of the drills will be kept on file in the FDH.
