54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted with the provider, an assistant, and ten children. The ages of the children range from five months old to five years old for a total of 24 points. Two household records and five children's records were reviewed. The children were having free play during the inspection. The provider is current in First Aid, CPR, and Medication Administration Training (MAT). Start time: 9:30 a.m. End time: 11:30 a.m.
Standard #: 22VAC40-111-60-B Description: **Repeat Based on a review of five children's records, the provider failed to maintain a complete record for three children's records. Evidence: Child A did not have the name, address, and telephone number for two emergency contacts. Child B was missing the house number of the street address listed for the second emergency contact. Child C was missing the street address for the first emergency contact listed. Plan of Correction: The provider will request the missing information from the parents today.
