Standard #: 22VAC40-111-920 Description: Based on observation of the provider meals, the provider did not have a menu posted which reflect the current one-week period. Evidence: The menu posted was dated for the week of 5/2-5/6 however it did not reflect the current one-week period. Plan of Correction: Provider stated that she will add the date.
Standard #: 22VAC40-111-320-B Description: Based on observation of the physical plant, electrical outlets accessible to children under five years of age were found missing protective covers. Evidence: In the nap room, two electrical outlets were missing protective covers. Plan of Correction: Provider covered during this inspection.
Standard #: 22VAC40-191-40-D-1-b Description: Based on review of the provider records, one caregiver record did not include documentation of a criminal record and central registry check before 30 days of employment ends. Evidence: Caregiver #2's who was hired on 9/2 did not have documentation of criminal record check results. Plan of Correction: Provider stated that she received the criminal record back however it did not include the results because she filled out something incorrectly on the form.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review of the provider records, one household member record did not include documentation of an updated criminal record, and central registry check three years from the date of the last screening. Evidence: Household member #1,#2 & #3's record did not include an updated Criminal record, cps check and sworn disclosure statement since 4/15. Plan of Correction: Provider stated that she mailed the forms however she did not submit enough money so she is awaiting the results.
