54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted on 04/5/2016 from 10:45 am - 11:45 am . There were 6 children present observed with the provider and one assistant. The children were observed in a variety of activities to include: free play, personal hygiene routines and eating lunch. There was a child that wasn't feeling well and was being comforted until his parent arrived. The following areas were inspected during today?s visit: physical environment, emergency preparedness plan, emergency drills, records ( children and caregiver) emergency supplies and posted documents. Standard #: 22VAC40-111-800-B Description: Based upon record review, required update was not completed. Evidence: The last annual review documented for the Emergency Plan was in 2013. Plan of Correction: The provider will complete review with assistant and document on the form hanging on the board.
Standard #: 22VAC40-111-60-B Description: Based upon record review, required information was found to be incomplete. Evidence: Child A's record is missing documentation that Proof of Birth was reviewed by provider within 7 days of first date of attendance. Child B's record is missing the medical insurance policy number. Child C's record is missing an annual review. Original paperwork signed 01/06/2015. Plan of Correction: Provider will ask parents for information at pick up.
Standard #: 22VAC40-111-320-B Description: Based upon observation, electrical outlets were not covered as required. Evidence: There were a total of 5 outlets that were not covered in the child care play area. Plan of Correction: A child in care keeps taking outlet covers out and putting in her mouth. Provider will purchase a different type of cover.
Standard #: 22VAC40-111-550 Description: Based upon observation, pack and plays and play pens cannot be used for sleeping infants. Evidence: A 4 month old infant was observed to be sleeping in a pack and play in the dining area next to the child care area. Plan of Correction: All infants will be placed in a crib to sleep. Provider will purchase new crib this week.
