Technical Assistance:
Pack and Play was observed not being used. The provided stated it is only used for activities only and no children sleep in it. Discussed the requirements to have a substitute provider.

Today an unannounced monitoring inspection was conducted from 4:00pm to 5:15pm. There were 10 children (24 points) in care with 2 caregivers. The children were observed in circle time, playing in stations, and preparing for dismissal. There were age appropriate supplies and adequate space. There are currently no medications on site, for children enrolled. An exit interview was conducted at the conclusion of this inspection. Should you have any questions please call.

Description: Based on review of records all children's files four children (#5, 7, 10, and 11) did not have all required documentation in their record. Evidence: Four children's records were missing documentation of annual review of emergency contact information. Records were last updated: #5-11/27/14, #7-1/2/14, #10-4/4/14, #11-9/10/14.

Plan of Correction: I will update the records tomorrow and will continue to update annually.
