Comments:
An unannounced renewal inspection was conducted on 6/3/2016 . There were two children in care on the day of the inspection. The inspector arrived at 12:05 P.M. and departed at 2:00 P.M. The inspector reviewed two child's records, the licensee's record and a household member's record. Observed the children during: nap, lunch and free play. Discussion with licensee about: night time care requirements for children, menu, and ensuring that the emergency preparedness plan is updated in January 2017. No medications are currently given.

Violations:
Standard #: 22VAC40-111-90-C
Description: Based on review of two children's records, the licensee failed to ensure that an immunization record was updated for all children. Evidence: 1. The inspector found that child # 2 did not have an updated immunization. The most recent immunization was dated 4/14/2015. For children under the age of two years it is required to update the immunizations every six months.

Plan of Correction: The licensee will speak with the parent about the updated immunization record. The provider thought that she had a copy and let the local agency have it for a copy and never received it back.

Standard #: 22VAC40-111-240-A
Description: Based on observation the house, the licensee failed to ensure that all areas and equipment were clean, safe and operable condition. Evidence: 1. The inspector observed an ash tray in the kitchen sitting on a hutch that was full of cigarettes and was accessible to the child in care.

Plan of Correction: The licensee removed and discarded the ash try during the inspection. The licensee will make sure in the future that it is not accessible to the children in care.
