Technical Assistance:
We discussed the requirements for an emergency radio, and training records. The risk assessment for a violation was provided. You can print model forms from the website dss.virginia.gov. We talked about your emergency response procedures.

Comments:
Thank you for your assistance during today's unannounced renewal inspection conducted from 12:35 PM to 5:00 PM. Today, there were twelve children in care at any time (36 to 37 points) with three caregivers. I viewed daily routines, activities, equipment, staffing, supervision, interactions with children, formula, nutrition/menu, snacks, diapering procedures, diaper ointment, sunscreen, posted information, emergency supplies and drill records, five records for children, three records for caregivers, caregiver training, indoor and outdoor areas. There continues to be care areas for the varying age groups and adequate age appropriate equipment, toys and materials. The caregivers were observed providing attentive supervision and positive guidance. The children received comfort and individual attention as needed. The children have enjoyed frequent outdoor play and experiences. Issuance of a renewed license will be recommended. Let me know if you need any assistance. 540/430/9259

Violations:
Standard #: 22VAC40-111-770
Description: Based on a review of the emergency supplies, and an interview with the provider, the battery-operated radio did not include the weather band station. Evidence: The previous battery-operated weather band radio was discarded due to a problem with the attery compartment. The battery-operated radio in the home only tunes to AM and FM stations and not a weather station.

Plan of Correction: During the inspection, the provider ordered a battery-operated weather band radio on-line to be delivered to the home. In the future, the provider will make sure there is always an operable radio that includes a weather band station.

Standard #: 22VAC40-111-230-A
Description: Based on a review of training records for three caregivers, and an interview with the provider, there was incomplete documentation of the required sixteen hours of training for 2015. Evidence: During the last inspection, there was eight hours of documented training for caregiver 1, 2 and 3. The documentation of any additional annual training in 2015 could not be located in the files. Also, the annual emergency response training for 2015 was not available in the files.

Plan of Correction: The provider will make sure all training certificates and documentation of annual training are kept in the designated files for all caregivers. The annual emergency training completed each September will be documented.
