- Technical Assistance:
The Inspector reviewed the chimney service invoice and could only make out that it was cleaned but no indication of the inspection of the device. Inspector consulted Provider to contact the company to request document to verify that the device's inspection was completed.
A renewal inspection was conducted from 10 AM - 1155. There were 5 children present with the Provider and assistant, for a total of 12 supervision points. The Inspector reviewed documents and observed program. The interaction between the Provider and children included play, colors, and centers. The Inspector talked over the phone with the back up assistant and items were discussed regarding misunderstanding with background checks. The Inspector has requested a phone call with background checks are returned so a review of documents can be made. Please call with questions 703-479-4679.
Standard #: 22VAC40-111-60-B Description: Based on record review of 3 children's files, the Provider has not maintained complete files for the children enrolled. Evidence: 1. Child 1's file (birth date September 18, 2013) is missing a signed statement from parent to acknowledge Provider does not maintain Liability Insurance. Provider was asked if she could locate this form and was unable to find it. Child 1" file also did not contain the Physical report and immunization record was last updated 20 months ago. 2. Child #2's file (birthdate October 24, 2014) is missing one of the two required emergency contacts. 3. Child #3's file (birthdate June 5, 2014) was enrolled on May 2, 2015 and the yearly review and parent signature to state items in file are still current has not been completed. It also has an immunization record that was last updated June 11, 2015. Plan of Correction: We have talked with the families and will again request the information
Standard #: 22VAC40-111-210-A Description: Based on record review of training documents, the assistant is 6 hours short of the annual 16 hours of training requirement. Plan of Correction: I will have everyone take 16 hours a year.
Standard #: 22VAC40-111-690-D Description: Based on staff interviews, and observation, the Provider is not cleaning and disinfecting the diaper changing pad according to the standards. which requires washing with soap and water and spray disinfectant that air dries. The Provider brought out the nonabsorbent pad to show the Inspector and when asked how she cleans and disinfects the pad, she said she uses baby wipes and then showed the Inspector Lysol brand wipes. Plan of Correction: We will correct this.
Standard #: 22VAC40-191-40-D-1-c Description: Based on record review and staff interviews, the Provider has not obtained for herself, assistant (also a household member), and her back up assistant before three years since her last background clearances were obtained. These include search of central registry and criminal history record check for each person. Evidence: 1. The Provider was cited for the background searches again on March 14, 2016 for not having documents. The Provider had Fairfax County Background search but the requirements state it is a State Police criminal history check. The central registry checks were also in violation because checks had not been obtained. 2. During today's inspection, the Provider stated when the call to a clearance office was made, she found out the birth date recorded was in question because it was crossed out and the correct date put in. She said she had the other ones and the forms she gave the Inspector were the Fairfax County Criminal History record which is not accepted. 3. All three staff still do not have up-to-date criminal history and central registry checks. Plan of Correction: Items have been sent and the Providers CPS had to be resent due to a misprint. We will also complete the other documents as the process has been confusing regarding which CR check should be sent but will have it done.
