Nine children were in care during the inspection. total of 19 points. The age range of the children was 19 months to four years. The provider and one assistant were present. The children were resting during the inspection. Two household and five children's files were reviewed. Inspection time 12:45pm-2:30pm

Standard #: 22VAC40-111-60-A
Description: Based on review of five children's files not all were complete. Evidence: Child A with a start date of 3/11/16 was missing address of two emergency contact and insurance policy number. Child B with a start of 2/23/15 was missing physical and work address for mother and father. Child C was missing one emercay contact and the insurance policy number.

Plan of Correction: We will ask the parents for the missing information.

Standard #: 22VAC40-111-430-A
Description: Based on observation there are no safety gates on stairs with children under the age of two in care. Evidence: There was no safety gate at the top of the stairs with a child under under the age of two in care.

Plan of Correction: We will install a gate this week.
