Technical Assistance:
Discussed with provider: all requried paperwork for children's files, paperwork for potential assistant, gate on the stairs.

An announced Initial inspection was conducted today. There are 2 children present (4 points) with 2 adults. 3 adult and 2 children's files were reviewed. See violation notice form missing items. There is one crib assembled for use with a manufacture date of 4/2016. Children were observed playing during today's inspection. Due to case load adjustments, your next inspection will be with a different inspector. All inspectors carry ID badges or a business card for security purposes. Description: There is not a second triangular bandage in the first aid kit.

Plan of Correction: I will make a second triangular bandage.

Description: The radio available for inspection is not a weather band radio.

Plan of Correction: I will purchase a new radio.

Description: The emergency preparedness plan is incomplete. Evidence: All required components of the plan have not been written in on the model form.

Plan of Correction: I will complete the model DSS form.

Description: Based upon a review of files, children's files are incomplete. Evidence: 2 of 2 files were reviewed were missing required information. Missing information includes but may not be limited to: *Parent's work information (child 1,2) *Health insurance information (child 1,2) *Emergency contact information (child 1,2) *Emergency care authorization (Child 1)

Plan of Correction: I will have the parents complete the model DSS forms.

Description: Based upon a review of files, there is no written acknowledgement of all required policies and procedures. Evidence: 2 of 2 children's files reviewed did not contain all policies and procedures. Missing information includes but may not be limited to: check in/out procedures, termination policy, DSS web site address.

Plan of Correction: I will complete the model DSS form and have the parents sign it.

Standard #: 22VAC40-111-320-A
Description: Based upon direct observation, small appliances were plugged in while not in use. Evidence: In the upstairs kitchen, a blender and food processor were plugged in while not in use.

Plan of Correction: Unplugged during the inspection. I will remind others to unplug them while not using them.
