Technical Assistance:
Consultation was given on using the basement for day care. The provider has applied for a zoning permit allowing for a capacity of 9 children.

Comments:
An unannounced monitoring visit was conducted during the Conditional licensure period. Three day care children and the provider's daughter were in care. Observed transition from lunch, preparation for rest time for the younger children and free play. Records for three children, emergency preparation, nutrition, physical space and infant care were observed. The provider uses the local park in the afternoons for large muscle playtime. Areas of non-compliance are identified in this report. Please sign and date the report and return it to me within 10 calendar days from date of receipt. Description: Based on record review, 3 of 3 children's records are incomplete. Evidence: Records for children were missing the following information. 1. Child #1's record was missing a complete emergency contact address and the first day of attendance. 2. Child #2's record was missing documented allergy or chronic problems asnd complete addresses for the two emergency contacts. 3. Child 3's record was missing a complete emergency contact address and parent work phone number.

Plan of Correction: Provider has requested all information from each parent.
