Technical Assistance:

An announced inspection was made on 6/23/16 at 9:30 a. m. and concluded at 12:30. The inspector met with the provider and Family Day Home standards were reviewed. There were three family day home children plus three provider's children present, with points equaling 4. The provider was caring for the children today. The children were observed having free play, including playing games, cards and music. The home has age and stage appropriate materials and equipment available for the children. The provider is going to use the model forms for children records. (One model sample record was reviewed) Two household member records were reviewed. Conversations were held with the provider and all of the children in care. One observation was conducted. The provider's policy is not to administer any medications.

Description: Based on review of emergency supplies the family day home did not have all of the required supplies. Evidence: On 6/23/16, the battery-operated weather band radio was not operable because there were no batteries available.

Plan of Correction: I will buy batteries tonight and will have them available for the next inspection. This will be fixed on 6/23/16.

Standard #: 22VAC40-111-800-A
Description: Based on discussion and review of paperwork the family day home did not have a required written emergency plan. Evidence: On 6/23/16, there was no written emergency preparedness and response plan for emergency evacuation, relocation and shelter-in-place procedures specific to the provider's home.

Plan of Correction: I will be filling out tonight and faxing over to DSS in the morning.

Standard #: 22VAC40-111-500-C
Description: Based on observation the family day home did not have all requirements in place regarding the playground area. Evidence: On 6/23/16, the inspector observed the playground equipment with monkey bars and swings placed over dirt. There were several play climbers and other toys outside on the playground placed closer than six feet apart.

Plan of Correction: I will call around today to get a price for mulch and this will be fixed as soon as possible. Children will not use the monkey bars and swings until fixed. I will remove climbers on 6/23/16 to allow the six feet of space between equipment.
