Technical Assistance:
Discussion of current staffing, residents, current enrollment, and the licensing process. Discussion of there being no bathroom downstairs in the day care area, but upstairs on first floor. All hand washing must also take place there as well.

The following inspection took place between the hours of 10 am through 12:30 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Ashburn, Virginia. At the start of this announced initial inspection there were a total of 5 children (13 points) with 1 caregiver. A total of 5 children?s files and 3 staff/household member?s files were reviewed. The children were engaged in the following: music, dance, learning, toy play, clean up, getting ready for lunch, and other organized activities. If you should have any future questions, please call. Standard #: 22VAC40-111-790
Description: Based on observation, the following telephone numbers were not posted: the responsible person for emergency backup care as required in 22 VAC 40-111-800 A 3; and 911. Evidence - 1. On the date of inspection (06/29/2016), only the number for poison control was posted. 2. The number of a back up was contained in the Emergency Preparedness and Response Plan (EP&RP) but not posted.

Plan of Correction: Both of these numbers will be posted.

Standard #: 22VAC40-111-920
Description: Based on observation, the posted menu was not dated. Evidence - 1. On the date of inspection (06/29/2016), the menu posted reflected general weeks of the month, but not a specific date. 2. The menu was observed on the Family Day Home's (FDH's) bulletin board.

Plan of Correction: The menu will have the specific date for each day.

Standard #: 22VAC40-111-100-A
Description: Based on review, 1 of 5 children files reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (06/29/2016), Child #4 was without documentation of a physical examination. 2. The start date of Child #4 was listed as 08/31/2015.

Plan of Correction: A physical shall be obtained for Child #4 and placed on file.

Standard #: 22VAC40-111-60-B
Description: Based on review, 1 of 5 children's files reviewed indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (06/29/2016), Child #5 was without the number to their insurance policy. 2. The policy name was included, but not the policy number.

Plan of Correction: The policy number shall be obtained and placed on file.
