An unannounced monitoring inspection was completed on 7/8/16. There were two children in care at arrival of the inspector with the provider supervising. Two additional children arrived at 12:15 pm. The point total was four with all children present. The ages were from seven years through nine years old. Seven children's records, the provider's record, and two household records were reviewed. The inspector arrived at the provider's home at 11:30 am and departed at 2:00 pm. The children were observed during a nap, during lunch, and during free time. There was discussion with the provider about the following: storage of background check records (locked file cabinet), updating information in the childrens' files to include changing grade level, ensuring the field trip permission forms are complete when school transportation begins (location/address not noted on several of the permission forms), two children missing first dates of attendance (documented during the inspection/were siblings of other children who started on same date), the renewal process for next inspection, the special use permit process through the local government to increase the license capacity (health department has approved the septic system and the zoning department will follow to give a permit), the process to modify the current license to increase the capacity if/when it is approved by the local government to care for more than five children.
Standard #: 22VAC40-111-830-A Description: Based on observation and interview with the provider, the provider failed to ensure that emergency evacuation drills were practiced monthly. Evidence: 1. The provider was licensed as of June 6, 2016. There was no documentation of a fire drill (emergency evacuation) practiced for the month of June 2016. According to the provider there were children in care during the month of June; however, she was waiting for all children to be present before a drill was practiced. Plan of Correction: The provider will ensure a drill is practiced monthly and documented in the future.
Standard #: 22VAC40-111-50-A Description: Based on observation and interview with the provider, the provider failed to ensure that there was a written record of children in attendance on the date of the inspection and the day prior to the inspection. Evidence: 1. The attendance for July 7 and July 8 were blank. According to the provider, there were children in care yesterday (July 7). During the inspection two school-aged children were observed in care from approximately 8:00 am. The attendance was observed at 12:00 pm and there were no children listed present. Plan of Correction: This was corrected during the inspection. The provider completed the attendance for the four children in care. In the future, the provider will ensure that the attendance is completed within 30 minutes of children arriving and marked off within 30 minutes of departure.
Standard #: 22VAC40-111-70-A Description: Based on record review, the provider failed to ensure that seven of seven children's records contained complete procedures required to be given to the parents in writing. Evidence: 1. All seven children's records were missing complete procedures for checking in and checking out. There was no daily schedule for the age of the enrolling child provided to parents on the "Information for Parents" model form. Plan of Correction: The provider will complete the procedures on the model. According to the provider, the parents are supplied a copy of a separate detailed agreement that lists times of the daily schedule and other procedures of the family day home. This is given to the parents at enrollment to keep for their records. In the future, the provider will make sure the "Information for Parents" form is complete that the parents sign to acknowledge receipt.
