The licensing inspectors conducted an unannounced monitoring inspection from 10:15am - 12:45pm. There were six children in care for a total of 14 points with two caregivers. The children were observed playing outside, during lunch and at nap time. One caregiver's record and two children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-111-100-A Description: Based on record reviews, the family day home failed to obtain documentation of a physical examination. Evidence: 1. The inspector reviewed two children's records. One of two records reviewed (child record #2) did not contain documentation of a physical exam and child's date of enrollment was 7/13/15. Plan of Correction: The provider will remind the child's parent that a copy of the child's physical is needed for the files.
Standard #: 22VAC40-111-60-B Description: Based on record reviews, the family day home failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed two children's records. One of two records reviewed (child record #1) did not contain the following: two emergency contacts, who can pick up the child and allergies and any health problems. One of two records reviewed (child record #2) contained only one emergency contact. Plan of Correction: The provider will request this information from the children's parents tomorrow.
Standard #: 22VAC40-111-70-A Description: Based on record reviews, the family day home failed to provide the parents all required documentation. Evidence: 1. The inspector reviewed two children's records. Two of two records reviewed (children's records #1 and #2) did not contain the family day home's policy for the administration of medications and the provisions of the home's emergency preparedness plan. Plan of Correction: The provider will give these documents to the children's parents for their review and signature.
Standard #: 22VAC40-111-180-A Description: Based on record reviews, the family day home failed to obtain a repeat tuberculosis screening. Evidence; 1. The inspector reviewed one caregiver's record. Caregiver record #1 did not contain a tuberculosis screening. Plan of Correction: The provider said that the tuberculosis screening for caregiver #1 has been misplaced. If the screening can not be found the provider will have caregiver #1 request another copy from the health department.
Standard #: 22VAC40-191-40-D-1-c Description: Based on record reviews, the family day home failed to obtain repeat background checks. Evidence: 1. The inspector reviewed one caregiver's record. Caregiver's record #1 had a search of the central registry that was date 8/29/12 and a sworn disclosure statement dated 7/20/12. Plan of Correction: The provider has misplaced the new search of the central registry check for caregiver record #1. The provider will try to find this form and if it cannot be found will be requested again. Caregiver #1 will complete a new sworn disclosure statement.
