Comments:
An unannounced monitoring inspection was conducted on 7/13/2016 from 9:50am - 10:40am. At the time of entrance there were three nonresident children in care along with two resident children. The ages of the children counted for a total of 12 points. Two caregivers were present. Children were observed during morning nap and morning snack which consisted of apples and milk. Hand washing was reviewed. The provider was observed providing care and comfort to one awake infant. The inspector additionally reviewed the following: records for three children, records for both caregivers and all household members, emergency supplies, the posted menu, written emergency plans, diapering procedures, the backyard play area, attendance documentation, annual training and the downstairs areas of the home. *No medication is being administered at this time.

Description: Based on record review and interview, the provider failed to document in writing the annual review and update of the emergency plan. Evidence: 1. The emergency plan for the family day home was last updated on 1/3/2015 according to written documentation in the plan. 2. The provider stated the plan had been reviewed and updated on 6/3/2016 however, this had not been documented in writing on the plan.

Plan of Correction: The annual review and updated will be noted on the plan today.
