Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:15am - 11:30am. There were six children in care (one residential) for a total of eight points with one provider. The children were observed playing with blocks, playing with play dough and watching television. One caregiver and three children's records were reviewed. Violations from the previous inspection were also reviewed and all had been corrected. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-111-500-C
Description: Based on observation, the family day home failed to have outdoor play equipment installed over an acceptable surface. Evidence: 1. The inspector observed the outdoor play equipment. There was bare ground under two of the swings.

Plan of Correction: The provider will purchase mulch and have it spread on the outside play area. The provider will also purchase mats to put under the swings.
