Comments:
An unannounced inspection was conducted on July 13, 2016. The inspection began at approximately 12:40 p.m. and ended at approximately 2:10 p.m.. The following areas were reviewed as a part of the inspection children's records, household member records, the physical environment, health and safety items, the menu and equipment and supplies were also reviewed. The provider is now using the backyard as their outdoor play space. There are two children currently enrolled however, there were no children in care during this inspection. Areas of noncompliance are noted on the violation notice. The results of the inspection were reviewed with the provider during the exit interview . Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation and provider interviews, the licensee did not ensure all areas and furnishings of the family day home, inside and outside, were maintained in a clean, safe, and operable condition. Unsafe conditions include, but are not be limited to, the presence of poisonous plants; tripping hazards; unstable heavy equipment, furniture, or other items that a child could pull down on himself; splintered, cracked, or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting, or breakage of any equipment; head entrapment hazards; and protruding nails, bolts, or other components that could entangle or could snag skin. Evidence: During the inspection on July 13, 2016, the following areas were observed in the outdoor area and poses a potential health and safety risk to the children. 1) Behind the deck in the open area there are 5 holes that measure approximately 2' 3'' by 2' 7' wide. The provider stated the holes were there because the holly bushes were removed. These hole pose a tripping hazard. 2) The wooden deck has areas of splintered and cracked wood. 3) There are 2 areas of vinyl siding on the outside of the house that are approximately 4 inches long where the vinyl is broken and has sharp edges.

Plan of Correction: The provider will fill the holes, the deck will be off limits until it is fixed and the provider will put something over the edges of the vinyl siding.
