An unannounced monitoring inspection was conducted on 7/21/16 from 3:15pm - 4:35pm. At the time of entrance there were 8 children ages two years old to five years old (16 points) in care with one caregiver. A tour of the home was conducted and children were observed playing inside and outside with toys. Records were reviewed for five children and four caregivers. There is no medication at the family day home. Emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-111-60-B Description: Based on a review of five children's records, it was determined that the provider did not ensure that they maintain and keep at the family day home a complete record for each child enrolled that contains all required information. Evidence: 1. The record for child #1, present during the inspection, did not contain documentation of the following items: the address and telephone number for the father; the name, address, and telephone number of the child's physician; the name and policy number of the child's medical insurance. The record also only contained documentation for one of the two designated persons to call in case of an emergency if the parent cannot be reached. 2. Caregiver #1 confirmed the record for child #2 was not complete. Plan of Correction: Child #1 just enrolled and the parent is in the process of completing all the missing information. All children's records will be complete prior to enrollment.
Standard #: 22VAC40-111-90-A Description: Based on a review of five children's records, it was determined that the provider did not ensure that before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations. Evidence: 1. The record for child #1, present during the inspection, did not contain documentation of an immunization record. 2. Caregiver #1 confirmed that there was no immunization record for child #1 that was available for viewing during the inspection. Plan of Correction: Child #1 just enrolled and the parent is in the process of obtaining a current immunization record. All children will have documentation of an immunization record when enrolling at the family day home.
Standard #: 22VAC40-111-180-A Description: Based on a review of four caregiver records, it was determined that the provider did not ensure that each caregiver has a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: 1. The record for caregiver #4 contained documentation for a tuberculosis screening that was completed on 2/13/14. 2. The provider confirmed that an updated tuberculosis screening had not been completed for caregiver #4. Plan of Correction: Caregiver #4 will get an updated TB screening. All caregivers will get an updated TB screening every two years.
