Please keep in mind to update pet immunizations in September 2016. LI Anderson reviewed online application instructions with the Provider. On or before Friday, July 22, 2016, the Provider will mail completed Renewal Application along with the appropriate fee to: VA Department of Social Services Division of Licensing Programs 210 First Street, Suite 405 Roanoke, VA 24011

An unannounced renewal inspection was conducted on 7/21/2016. Upon arrival at 9:35 AM, there were a five (5) children and the Provider present. Children present totaled five (5) points under the direct supervision of 1 caregiver. Children & staff/household records were reviewed during today?s inspection. The Provider will no longer administer prescribed nor over the counter medication & will not renew MAT certification. Provider has notified all parents in writing of the changes in medication administration. Provider continues to participate in the USDA Child Care Food Program and other county/state sponsored programs continue to be a part of the services offered to enrolled children & their families. Provider is now participating in the Smart Beginnings child care professional development program. Daily activities for the children include snacks/meals, quiet/independent activities, directed/group activities & outdoor play (weather permitting). Inspection concluded at 11:05 AM. LI Anderson conducted the exit interview with the Provider today which included review of the areas of non-compliance & Risk Assessment.

Description: Based on attendance record review, the Licensee failed to keep a written record of children in attendance each day. EVIDENCE: Daily attendance was not documented for July 19th & 20th.

Plan of Correction: Provider will document attendance each day. Corrected during inspection
