The licensing inspector conducted an unannounced focused monitoring inspection from 10:30am - 12:15pm. There were six children in care for a total of eleven points with one caregiver. The children were observed watching a movie and playing outside. Two children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Description: Based on observation, the family day home failed to maintain inside and outside the home in a clean and safe condition. Evidence: 1. The inspector observed the following: There was dirt, paper, a dead roach, a plastic fork and a plastic spoon on the floor in the entrance room which is primarily used for child care. Also in this room, the inspector observed a dead and a live roach on the table. Outside, the inspector observed the grass in the yard. The grass is several inches high in the play area.

Plan of Correction: The provider swept the floor during the inspection and removed these items from the floor. The provider showed the inspector the receipt for the exterminator who was at the home yesterday and sprayed for roaches. The provider cleaned and sanitized the table during the inspection. The provider said that the grass will be cut. The children will not play in this area until the grass is cut.
