54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring visit and home tour was conducted from 2:50pm-3:45pm. At the time of the inspector's arrival there were 6 children ages 5mos-4yrs, totalling 14 points in care with the licensee. Children were observed participating in free play, snack, and toileting/handwashing. A sample of 3 children's records and 1 staff record and 1 household member record were reviewed. Medication is not administered at this site. There were no reported injuries since the last inspection. Areas of noncompliance are identified on the violation notice and were discussed with the provider during the exit meeting at the conclusion of this inspection.

Standard #: 22VAC40-111-100-A
Description: Based on record review it was determined that the provider did not obtain documentation of a physical examination by or under direction of a physician for each child within the required time frame. Evidence: The record for Child #3 (enrolled 3/28/16) did not contain documentation of a physical exam.

Plan of Correction: Provider will request documentation of a physical exam from the parent at pick up.

Standard #: 22VAC40-111-180-A
Description: Based on record review it was determined that the provider did not ensure that each caregiver had a current report of Tuberculosis screening every two two years from the date of the first screening. Evidence: There was no documentation of a current Tuberculosis screening in the provider's record. The provider's last Tuberculosis screening was completed on 5/16/14.

Plan of Correction: Provider is going to get Tuberculosis screening done 07/27/16.
