The following inspection took place between the hours of 8:30 am through 10:00 am. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced renewal inspection there were a total of 9 children (27 points) with 2 caregivers. A total of 3 children's files and 5 staff/household member's files were reviewed. The children were engaged in the following: breakfast, diapering, handwashing and a walking trip outside. The violation was reviewed with the provider. Standard #: 22VAC40-111-60-B Description: Based on record review, three children records were incomplete. Evidence: Child #1 and #3 's record did not have documentation of the medical policy number. Child #1, #2 and #3 records did not have written authorization for regular scheduled trips. The children went on a walking field trip during this inspection. Plan of Correction: I will get the policy number from the parent. I will have the parents complete the form for walking trips.
