Comments:
Today an unannounced 2 month visit was conducted with the Provider. This visit took place in the morning between 10:00-12 there weer 2 children present with the Provider for a total of 8 points within the supervision guidelines. The children were observed having morning tummy time, listening to music, free play and working on using gross motor skills. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children records, fire drill log, cribs, and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-60-B
Description: Based on review of the children records, one record did not include documentation of two designated two or more emergency contact persons complete name, address and phone number. Evidence: Child #1's record, did not include the two designated emergency contact person's complete address.

Plan of Correction: Provider will call the parents and request the information today.
