Comments:
The following inspection took place between the hours of 11:00am through 12:30pm. At the start of this unannounced monitoring inspection there were a total of 8 children (totaling 26 points) and 2 staff. A total of 2 children?s files and 2 staff files were reviewed. This inspector also reviewed physical plant and emergency procedures and documentation. The children were observed having lunch, taking restroom breaks, hand washing and engaging in free play. Standard #: 22VAC40-111-960-C
Description: Based on direct observation, an infant was placed at a feeding table and was not fastened with the safety waist and crotch strap. Evidence: 1. Child #3 was placed at the feeding table and was not secured with the safety waist and crotch strap.

Description: Based on record review, 1 of 2 children's files were not complete. Evidence: 1. Child #1 was missing the address for both emergency contacts. 2. Child #2 was missing the child's medical insurance policy number, parental authorization to provide emergency medical care, and documentation of an annual review of emergency contact information.
