An unannounced inspection was conducted this morning. There was a caregiver present with 5 children for a total of 10 points and the Provider returned from an appointment during the inspection. The Program's set up allows movement and access to various materials. The children were engaged in free play and talked to the Inspector about starting school and toys. The Inspector reviewed attendance record and 3 children's files. Please call with questions 703-479-4679
Standard #: 22VAC40-111-100-A Description: Record review of child #1's file showed the documentation of a physical examination was not obtained. The file contains an immunization form but does not contain a physical report. Plan of Correction: I will request the documents again.
Standard #: 22VAC40-111-690-D Description: Based on observation, the changing surface is an absorbent surface and cannot be cleaned and sanitized after each use. Plan of Correction: I removed cloth cover during the inspection.
