Comments:
Thank you for your time and assistance with the unannounced inspection conducted today, August 24, 2016. The on-site visit was from 11:00 a.m. to 1:00 p.m. On this date, there were four (4) children in the care of the provider for a total of eight. There are a total of then (10) children enrolled in this family day care home. Four children's records were reviewed. Two staff and one household record were reviewed and updated. Areas of non-compliance may be found on the violations notice. Risk scores were discussed. All interactions between the provider and the children were appropriate and the children seemed comfortable with the daily routine. This family day home is well equipped with a variety of age/stage materials for the children in care. Description: Criminal history check was not updated for one household member. Evidence: Based on record review and conversation with the provider it was confirmed that an updated criminal history check was not updated for one household member. Last documented update was 05/14/2013.

Plan of Correction: An updated criminal record check will be completed. Updates will be done every three years.
