Standard #: 22VAC40-111-60-B Description: Based on record review, the provider did not obtain the policy number of the child's medical insurance and the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached for one child in care. Evidence: Child 2 did not have a second emergency contact listed in her record and also did not have a medical insurance policy number. Plan of Correction: I will ask the parent for the information and complete the form as soon as possible.
