Description: The provider failed to record the hours a substitute provider was caring for the children. Evidence: Based on the interview with the provider and assistant, the provider has not been recording the hours covered by the assistant when the provider has been out of town or on vacation. The provider indicated that she did not know that was something she had to do.

Plan of Correction: I will make sure I record the hours when a substitute is used and that the substitute records and signs the form as needed.
