Technical Assistance:
Procedures for walking filed trips reviewed.

An unannounced monitoring inspection was conducted on 9/6/2016 from 10:00am - 11:05am. At the time of entrance there were three children in care with the provider. The ages of the children counted for a total of eight points. Children were observed playing with toys at the table with the provider. Records were reviewed for the three children in care as well as the provider and assistant. The inspector additionally reviewed emergency supplies, the posted menu, diapering and hand washing procedures, the backyard play area, the child care room documentation of emergency practice drills, attendance documentation and the written emergency plan for the FDH.

Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the provider failed to ensure that the first aid kit contained a working digital thermometer. Evidence: The digital thermometer in the first aid kit was not working.

Plan of Correction: A new thermometer will be purchased today.

Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider failed to annually review with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review. Evidence: The most recent review of the information in the record of child #3 was done on 3/23/2015.

Plan of Correction: The parent of child #3 will be asked to review the enrollment record this afternoon. Documentation will be noted in the child's record.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the provider failed to obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The most recent immunizations on file for child #1 were administered on 7/30/2015. Child #1 is under the age of two years. 2. The provider confirmed the records on file as the most recent provided by the parent of child #1.

Plan of Correction: The parent will be asked today to provide a copy of updated immunizations for child #1.
