An unannounced renewal study was conducted with the provider from 10:20 a.m.-11:45 a.m. Eight children, ages 2 years, 3 years, 3 years, 4 years, 4 years, 4 years, 4 years, and 4 years old, were in the care of one caregiver for a total of 16 points. Children were observed building with blocks, playing with toy cars, watching a cartoon, and playing a toy guitar. Lunch served today will be corn dog, macaroni and cheese, green beans, fruit cup, and milk. The last emergency escape drill was 9/12/16. The complete first aid kit, weatherband radio, and flashlights were present. Two household members' records were reviewed. Two children's records were reviewed and complete. Standard #: 22VAC40-111-920
Description: Based on observation and provider interview, the provider did not have a menu for the current one-week period. Evidence: There was no menu available for the current week of September 12, 2016.

Plan of Correction: The provider stated that she will complete the menu for the current week.

Standard #: 22VAC40-111-180-A
Description: Based on review of two household members' records, the provider did not obtain updated tuberculosis (tb) tests or screenings for each household member every two years. Evidence: The provider's and Household Member A's last tb tests were dated 7/27/14.

Plan of Correction: The provider stated that she will schedule tb tests for her and Household Member A.
