An unannounced monitoring inspection was conducted on Monday, September 12, 2016, from approximately 10:00am until 12:45pm. Present today were ten children totaling thirty-two points with three caregivers. Children were observed during circle time, lunch and nap. They sang several songs, listened to stories and played with a variety of manipulatives. Lunch: Turkey, croissant, corn, applesauce and milk The last practice evacuation drill was conducted 8/19/16. Two children's records and one caregiver record were inspected.
Standard #: 22VAC40-111-60-A Description: Based on review of children's records, the provider did not ensure all required information was contained in each child's record. Evidence: Out of two children's records inspected, Child #2 record was missing a second emergency contact including name, address and phone. The one emergency contact listed is missing the phone number. Plan of Correction: Per Provider: " Will get information completed today."
