Technical Assistance:
The license cannot be processed for renewal until the provider forwards documentation of the remainder of her training to the Inspector.

Comments:
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 3 children, totaling 9 points. The children were observed playing with toys, interacting with each caregiver, and preparing for lunch. The children were to be served ham and cheese sandwiches with lettuce and tomato, bananas, and milk for lunch today. This inspection was completed between approximately 10:15 am and 11:40 am. Violations:
Standard #: 22VAC40-111-730-D
Description: Based on observations, it was determined that one adult non-prescription medication was not kept in a locked place. Evidence: One container of non-prescription pain medication was stored on the counter in the kitchen.

Plan of Correction: The medication was immediately moved during the inspection. The medication belongs to a friend of mine and she is picking it up today. I left it out to remind myself to give it to her when she comes.

Standard #: 22VAC40-111-800-B
Description: Based on a review of the emergency preparedness plan, it was determined that the provider has not conducted an annual review of the plan. Evidence: The provider most recently reviewed her emergency preparedness plan on 4/15/15. This review expired on 4/15/16.

Plan of Correction: My assistant and I will review the plan today during the children's nap time.

Standard #: 22VAC40-111-920
Description: Based on a review of administrative documents, it was determined that the provider did not update the posted menu as required. Evidence: The provider did not update the posted menu to indicate the substitute foods being served for lunch today.

Plan of Correction: I will remember to update the menu when necessary.

Standard #: 22VAC40-111-960-B
Description: Based on observations, it was determined that one infant was not held when bottle feeding. Evidence: One 4 month old infant was observed drinking from her bottle while seated in the infant bouncy seat. Since the child is too young to hold her own bottle, the bottle was propped on a blanket

Plan of Correction: The baby was immediately taken out of the seat and held to finish her bottle. All babies will be held if they are too young to hold their own bottles.

Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1. Child record #1 does not include a second emergency contact other than the parent; the parent's medical insurance policy number. 2. Child record #2 does not include the name and policy number of the parent's medical insurance. 3. Child record #3 does not include a second emergency contact other than the parent.

Plan of Correction: The parents will be asked for all missing information.

Standard #: 22VAC40-111-130
Description: Based on interviews and observations, it was determined that the current assistant caregiver is not fully qualified for her position. Evidence: The assistant caregiver was unable to communicate with the Inspector. When asked basic questions, such as "How many children are here today?" and "What would you do if there was a fire?", the assistant caregiver was unable to answer the questions.

Plan of Correction: She is already never left alone with the children. I will continue this. She's currently enrolled in English speaking classes. I will work with her to make sure her English skills improve.

Standard #: 22VAC40-111-320-I
Description: Based on observations, it was determined that the smoke detector in the child care area is not working properly. Evidence: During this inspection, the smoke detector repeatedly beeped to indicate that the battery needs to be replaced.

Plan of Correction: I will replace the battery today.

Standard #: 22VAC40-111-460-I
Description: Based on observations, it was determined that the dog's dishes were not kept inaccessible to the children. Evidence: Upon arrival, the dog's water bowl and filled food dish were on the floor in the child care area. These items were accessible to the children.

Plan of Correction: I forgot to move the bowls at the end of the weekend. The bowls were immediately moved during the inspection.
