Comments:
Conducted an unannounced renewal study with the provider, an assistant, and three children for a total of eight points. The ages of the children range from one years-old through four years-old. The children were eating lunch, having free play, and napping during the inspection. Three children's records and four household records were reviewed. The provider and the assistant are current in First Aid and CPR. The assistant is current in MAT (Medication Administration Training). There were no children on medication today. Start time: 12:00pm End time: 2:45pm

Standard #: 22VAC40-111-60-B
Description: Based on a review of seven children's records, three records were missing information. Evidence: Child A, B, and C were missing the name and policy number of the children's medical insurance information.

Plan of Correction: The provider stated that he will speak to the parents and get the missing information.
