Comments:
An unannounced monitoring visit was conducted today. Nine children were present with two caregivers for twenty eight points which is within ratios for supervision. Children were observed playing and listening to music in the playroom and three children were asleep in the nap room. The provider has Medication Administration Training (MAT) but no medication was reported to be on-site today. The assistant file was reviewed and found to be complete as well as one child's record. Emergency preparation is current for the written plan and drills. First aid and CPR certification is up to date. Lunch to be served today was chicken with veggies, cantaloupe, bread and milk. The household pets have updated rabies documentation. Please sign and date the report and return it to me within 10 calendar days from date of receipt.
