Technical Assistance:
Consultation was given on the need for the upstairs door to have a door knob in case the children needed a secondary emergency exit as well as trying to shorten the time to exit the home as the drills are currently taking approximately 8 minutes.

An unannounced monitoring visit was conducted during the Conditional licensure period. Three children were present with the caregiver and an assistant. The assistant must have all completed background checks and trainings (16.0 hours annually) even if she only works part-time. Children were observed playing in free play, having snack and reading. Lunch to be served today was mac and cheese, chicken nuggets, spinach and tomatoes. Emergency drills were up to date. Areas of non-compliance are identified in this report. Description: Based on record review, an assistant record is incomplete. Evidence: Assistant #1 is missing two reference checks prior to employment and an emergency contact. Assistant only works part time but was present at the June 2016 visit.

Description: Based on provider statement, the assistant has received orientation training but it was not documented. Evidence: Assistant was present during both the initial and 2 month monitoring visit but no documentation of orientation, signed by both the provider and assistant, stating her job responsibilities, health and safety procedures and policies for the home were reviewed by the end of the first week of working.

Description: Based on observation, an area of the home is not maintained in a safe manner. Evidence: The door at the top of the stairs is missing a door knob. If the door were to close and children needed to exit from the main level of the home there would be no way to open the door from the day care side.
