54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted today. Observed 8 children (30-points) directly supervised by 2 caregivers. Ratio and supervision were in compliance. A sample of 2 children's records, and 2 caregiver records were reviewed. The physical plant and emergency procedures were also reviewed. No medications were reported on site. The children were engaged in various activities, to include: free play, reading,music time and circle time; that included the "Good friend "song, show and tell and practicing writing the letter "A". The interior of the family day home was found to be clean, organized, and well supplied with toys and equipment for the children. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-830-C Description: Based upon observation, required documentation was not on record. Evidence: Required documentation of monthly emergency evacuation drills were not available at the time of inspection for the following months( 3/16, 4/16, 5/16, 6/16, 7/16, 8/16). Plan of Correction: Provider will start a new log to place the one that cannot be located.
Standard #: 22VAC40-111-60-B Description: Based upon record review, required documentation was found to be missing or incomplete. Evidence: Child A and Child B are both missing complete emergency contact information for contacts #1 and #2. Plan of Correction: Provider will ask parents for complete information.
Standard #: 22VAC40-111-240-A Description: Based upon observation, equipment in home needs to be repaired. Evidence: The towel bar in the children's bathroom was loose and the bracket and screws were hanging from the wall. Plan of Correction: Provider will remove until the bar can be re-hung.
