Description: Based on review of one caregiver's record and discussion with caregiver #1 on October 10, 2016, the facility failed to obtain a criminal history record check and a search of the central registry clearance before 30 days of employment ended. Evidence: 1. Review of caregiver #2's record, on this date, determined that background clearances (criminal history record check and search of the central registry) were not on file (hire date - 9/07/2016). 2. Caregiver #1 stated that the background request forms had been mailed but had not yet been received. The date of mailing was not documented.

Plan of Correction: The office of background investigation (central registry) and the state police (criminal history record) will be contacted to check on the status of the background request forms. In the future, background clearance request forms will be mailed within one week of an employee's hire date and the mailing date will be documented in the employee file.
